Form f 8718: User Fee for Exempt Organizations: Determination Letter Request

Use this form when applying to pay the $100 fee for not filing on time. Attach this form to the determination letter request.

Great Plains Leadership Institute 2013

This form includes information, driving directions, and an application blank for the 6th Annual Great Plains Leadership Institute to be held at the University of St. Mary in Leavenworth, Kansas July 11-13. Letter of Request: Asking for relief from high fees

This is a sample letter explaining why your club either missed the filing date or failed to file in the first place. Adapt the specifics in the letter to meet your needs. Including this letter may help your club qualify to pay only the $100 late fee, not the $400 or $800 fee or higher. Deadline is Dec 31, 2012 to request and file the other forms.
